ABSTRACT

OBJECTIVE@#To compare the safety differences between Chinese medicine (CM) and Western medicine (WM) based on Chinese Spontaneous Reporting Database (CSRD).@*METHODS@#Reports of adverse events (AEs) caused by CM and WM in the CSRD between 2010 and 2011 were selected. The following assessment indicators were constructed: the proportion of serious AEs (PSE), the average number of AEs (ANA), and the coverage rate of AEs (CRA). Further comparisons were also conducted, including the drugs with the most reported serious AEs, the AEs with the biggest report number, and the 5 serious AEs of interest (including death, anaphylactic shock, coma, dyspnea and abnormal liver function).@*RESULTS@#The PSE, ANA and CRA of WM were 1.09, 8.23 and 2.35 times higher than those of CM, respectively. The top 10 drugs with the most serious AEs were mainly injections for CM and antibiotics for WM. The AEs with the most reports were rash, pruritus, nausea, dizziness and vomiting for both CM and WM. The proportions of CM and WM in anaphylactic shock and coma were similar. For abnormal liver function and death, the proportions of WM were 5.47 and 3.00 times higher than those of CM, respectively.@*CONCLUSION@#Based on CSRD, CM was safer than WM at the average level from the perspective of adverse drug reactions.

ABSTRACT

Objective:Metabolomics was used to analyze the brain tissue samples of model mice with chronic unpredictable mild stress (CUMS) depression, in order to find out the differential metabolites related to depression and to explore the possible antidepressant mechanism of iridoid part of Valerianae Jatamansi Rhizoma et Radix (IEFV). Method:Forty-two Kunming mice were randomly divided into 6 groups, including the normal group, the model group, the fluoxetine group (2.5 mg·kg-1) and the IEFV low, medium, and high dose groups (doses were 5.73, 11.47, 22.94 mg·kg-1, respectively). The behavioral and biochemical indicators of CUMS model mice were used for pharmacodynamic evaluation with IEFV and a positive drug (fluoxetine) as the intervention drugs. Then, the effect of IEFV on endogenous substances of the brain tissue in CUMS model mice were analyzed by nuclear magnetic resonance hydrogen spectrum (1H-NMR) metabolomics, and multivariate statistical analysis was used to identify the differential metabolites and to enrich the metabolic pathways involved in the differential metabolites. Result:After modeling, the immobility time of the model mice increased significantly, their sucrose preference rate and the excitatory neurotransmitters [5-hydroxytryptamine (5-HT) and norepinephrine (NE)] decreased significantly, indicating the success of modeling. The depression was relieved after IEFV administration, mainly manifested by the recovery of the immobility time, sucrose preference rate and the excitatory neurotransmitters (5-HT and NE). Principal component analysis (PCA) of endogenous metabolites in brain tissue showed that the model group could be significantly separated from the normal group, while the IEFV groups and fluoxetine group all showed a trend of deviating from the model group to the normal group, which was consistent with the behavioral results. The results of orthogonal partial least squares discriminant analysis (OPLS-DA) showed that there were 16 different metabolites between the model group and the normal group, including 12 water-soluble metabolites and 4 liposoluble metabolites. Seven potential metabolism pathways were obtained through MetPA analysis, including metabolism of phenylalanine, metabolism of phenylalanine, tyrosine and tryptophan, metabolism of taurine and hypotaurine acid, metabolism of alanine, aspartic acid and glutamic acid, biosynthesis of valine, leucine and isoleucine, metabolism of D-glutamine and D-glutamate and tricarboxylic acid cycle (TCA). IEFV-high dose group could significantly recall 11 differential metabolites. Conclusion:IEFV may play an antidepressant role mainly by affecting energy metabolism, amino acid metabolism and neurotransmitter levels, which provides a reference for further study on the antidepressant mechanism of IEFV.

ABSTRACT

Objective:To discuss the effect of volatile oil from Xanthii Fructus on airway remodeling of rats with bronchial asthma, in order to discuss the mechanism of action through matrix metalloteinases-9 (MMP-9), tissue inhibitor of metalloproteinase-1 (TIMP-1), platelet derived growth factor (PDGF), transforming growth factor-β1 (TGF-β1), Smad2, Smad3 and Smad7 mRNA expressions. Method:Forty-eight rats (half males and half females) were randomly divided into six groups (n=8), namely normal saline group, model group, volatile oil from Xanthii Fructus (7.5, 15, 30 mg·kg-1) groups and dexamethasone group (0.5 mg·kg-1). Except for normal saline group, airway remodeling of rats were established through ovalbumin sensitization and atomization inhalation method. At volatile oil group, rats got volatile oil spray since the first time (the 4th week after modeling) of inspiration of asthma to the last day before the end of test. Histopathological changes of bronchus and lung were analyzed by htoxylin eosin (HE) stain. And airway remodeling was observed, and perimeter of bronchial lumen (Pi), smooth muscle area of bronchi (S), tube wall area (W) and nuclei of smooth muscle cells (N) were recorded. And levels of endothelin-1 (ET-1) and insulin-like growth factor-1(IGF-1) in irrigation solution of Alveoli (BALF) were detected. And mRNA expressions of MMP-9, TIMP-1, PDGF, TGF-β1, Smad2, Smad3 and Smad7 were detected by real-time quantitative-polymerase chain reaction (Real-time PCR). Result:In the model group, infiltration of a large number of inflammatory cells were found at the peribronchial wall, airway smooth muscle thickness and basement membrane increased, and bureaucratic cavity was narrower and flatter. Volatile oil from Xanthii Fructus can ameliorate pathological damage of bronchopulmonary tissue. Levels of S/Pi, W/Pi and N/Pi, BALF, ET-1, IGF-1 and MMP-9/TIMP-1 and mRNA expressions of MMP-9 and PDGF in volatile oil group and dexamethasone group were lower than those in model group (Pβ1, Smad2 and Smad3 were less than those in model group (PPConclusion:Volatile oil from Xanthii Fructus can regulate levels of MMP-9, TIMP-1, MMP-9/TIMP-1, TGF-β1/Smad, inhibit expressions of PDGF, ET-1 and IGF-1, and suppress subepithelial fibrosis, extracellular matrix synthesis, proliferation and migration of airway smooth muscle cells to relieve or inhibit airway remodeling, so as to prevent and treat asthma.

ABSTRACT

Objective: To observe the clinical efficacy of Jieyu Tongluo decoction on post-stroke depression (PSD) and its regulatory effect on neurotransmitter. Method: One hundred and twenty-five patients with PSD were randomly divided into control group (62 cases) and observation group (63 cases) by random number table. Both groups got psychological support, health education, and sertraline hydrochloride tablets, 25-50 mg/time, 2 times/days. Patients in control group got Shugan Jieyu capsules, 2 grains/time, 2 times/days. Patients in observation group got Jieyu Tongluo decoction, 1 dose/day. And a course of treatment was 6 weeks. Before and after treatment, hamilton depression scale (HAMD-17), patient health questionnaire-9 (PHQ-9), national institutes of health stroke scale (NHISS),traditional Chinese medicine(TCM) syndrome and activity of daily living scale (ADL) were scored. And levels of 5-serotonin (5-HT), norepinephrine (NE), dopamine (DA), brain-derived neurotrophic factor (BDNF), serum S100-β and neuron enolase (NES) were detected. Result: By rank sum test, after treatment, the clinical efficacy of observation group was superior to that in control group (Z=2.384, PPZ=2.266, PZ=2.817, Pβ and neuron enolase (NES) were lower than those in control group (PPConclusion: In addition to psychotherapy and SSRI, Jieyu Tongluo decoction can relieve degree of depression, depressive symptoms and degree of neurologic impairment, increase activities of daily living, regulate neurotransmitter expression, and improve the reparation of cranial nerve tissue, with obvious clinical effects, and is worthy to be used in clinic.

ABSTRACT

The prescription of clinical curative effect has promoted the formation and development of the dominant diseases in traditional Chinese medicine, but it has been controversial for a long time because its mechanism has not been effectively explained. Breaking the gap between animal/cell research and clinical research, and understanding the mechanism of dominant diseases in traditional Chinese medicine based on evidence-based medicine has become an important breakthrough in this scientific issue. Therefore, based on evidence-based medicine, we established the research concept that "originating from clinic, testing in experiment, returning to clinic". Taking the classic formula (Jinqi Jiangtang formula) treating diabetes as an example to find characteristic markers of diabetes supported by evidence-based medicine from clinic. We used the reverse analysis strategy of the response of characteristic markers to explore the intervention mechanism of Jinqi Jiangtang formula on characteristic markers. Then, we verified the key signaling molecules of the metabolic regulation of the Jinqi Jiangtang formula in clinic. The research ideas and key technologies for the mechanism of treatment of diabetes by Jinqi Jiangtang formula based on evidence-based medicine are formed, and it is expected to provide research reference for explaining the mechanism of dominant diseases in traditional Chinese medicine based on evidence-based medicine.

ABSTRACT

With the growth of number of Chinese patent medicines and clinical use, the rational use of Chinese medicine is becoming more and more serious. Due to the complexity of Chinese medicine theory and the uncertainty of clinical application, the prescription review of Chinese patent medicine always relied on experience in their respective, leading to the uncontrolled of clinical rational use. According to the traditional Chinese medicine (TCM) theory and characteristics of the unique clinical therapeutics, based on the practice experience and expertise comments, our paper formed the expert consensus on the prescription review of Chinese traditional patent medicine for promoting the rational use of drugs in Beijing. The objective, methods and key points of prescription review of Chinese patent medicine, were included in this expert consensus, in order to regulate the behavior of prescription and promote rational drug use.

ABSTRACT

Objective To discuss the stool coccobacillus flora features between lung diseases patients and non-lung diseases patients for confirming homotherapy for the lung and the large intestine ideology based on the lung and large intestine being interior-exteriorly related theory. Methods Totally 112 patients were divided into lung diseases group (47 cases) and non-lung diseases group (65 cases) by cross-sectional study. Questionnaire survey for patients was used, including general demographic information, lung diseases and course of disease, non-lung diseases and course of disease, medication, diet, sleep, emotional, physical and stool. Stool specimens were collected and smear method was used to detect the ratio of coccus. Results 3 cases were lost in the lung diseases group and 5 cases were lost in the non-lung diseases group. Stool coccobacillus mean proportionality was 0.78 in lung diseases group, and 0.35 in non-lung diseases group, with statistical significance (P=0.041). Stool gram negative bacilli mean value was 56.59% in lung diseases group, and 65.50% in non-lung diseases group, with statistical significance (P=0.040). Stool gram positive coccus mean value was 33.52% in lung diseases group, and 23.80% in non-lung diseases group, with statistical significance (P=0.004). There were 23 cases of stool gram negative bacilli aberrant type in lung diseases group, and 44 cases in non-lung diseases group, with statistical significance (P=0.038). There were 21 cases of loss of appetite in lung diseases group, and 14 cases in non-lung diseases group, with statistical significance(P=0.012). There were 23 cases of constipation in lung diseases group, and 19 cases in non-lung diseases group, with statistical significance (P=0.044). There were 15 cases of loose stool in lung diseases group, and 9 cases in non-lung diseases group, with statistical significance (P=0.033). Conclusion Lung diseases patients occur to alteration of intestinal flora potentially. And symptoms include loss of appetite, constipation, loose stool, which occur to lung diseases patients significantly, indicating lung and large intestine tend to exist homopathy. It is important to homotherapy for the lung and the large intestine based on the lung and the large intestine being interior-exteriorly related.

Objective:To analyze the main influencing factors of China's commercial health insurance demand and promote the development of China's health service industry.Methods:Based on the panel data of 31 provinces and cities in China from 2009 to 2015,the model of cross-sectional fixed effect was constructed to conduct empirical analysis on the main influencing factors of China's commercial health insurance demand.Result and Conclusion:More significant influences on commercial health insurance demand factors were urban per capita disposable income,per capita urban employee basic medical insurance fund income,elderly population bring-up ratio and per capita outpatient expenses.For the basic medical insurance of urban employees,the urban employee medical insurance had significant effect on the demand of commercial health insurance.For the basic medical insurance of urban residents,the income impact was more prominent.The proportion of graduates of general institutions of higher learning had few influence commercial health insurance.

ABSTRACT

Objective To investigate the clinical value of individual design combined with 3D-guided percutaneous hollow screw fixation for patients with Herbert Ⅰb-type scaphoid fractures.Methods Totally 93 cases of Herbert Ⅰb-type scaphoid fractures patients who were admitted into our hospital from November 2015 to April 2017 were divided into the external fixation group(31 cases),the internal fixation group(29 cases)and the minimal invasion group(33 cases).The external fixation group was treated with cast immobilization;the internal fixation group was treated with open reduction and internal fixation;the minimal invasion group was treated with individual design combined with 3D-guided percutaneous hollow screw fixation.The time of bone union,return-to-work time,rateof bone union,wrist range of motion (ROM)and Mayo function score of the three groups were compared.Results The time of bone union,return-to-work time and wrist range of motion in the minimal invasion group were(6.7 ±0.9)weeks,(7.3 ±0.9)weeks and(103.8 ±5.9)°respectively,which were better than the other two groups with statistically significant difference(P<0.05).The bone union rate of the external fixation group,the internal fixation group,and the minimal invasion group were 83%,86.2%and 100%respectively.The difference between the minimal invasion group and the other two groups were statistically significant(P<0.05).The Mayo function score of the external fixation group,the internal fixation group, and the minimal invasion group were 71.0%,82.8% and 97.0% respectively.The difference between the minimal invasion group and the other two groups were statistically significant(P<0.05).Conclusion Individual design combined with 3D-guided percutaneous hollow screw fixation treatment has exact effect for Herbert Ⅰb-type scaphoid fractures with fast recovery,and it's worthy of wildly use.

ABSTRACT

Abstract Engyodontium album is a widespread pathogen that causes different kinds of dermatoses and respiratory tract diseases in humans and animals. In spite of its perniciousness, the basic genetic and molecular background of this species remains poorly understood. In this study, the mitochondrial genome sequence of E. album was determined using a high-throughput sequencing platform. The circular mitogenome was found to be 28,081 nucleotides in length and comprised of 17 protein-coding genes, 24 tRNA genes, and 2 rRNA genes. The nucleotide composition of the genome was A+T-biased (74.13%). Group-II introns were found in the nad1, nad5, and cob genes. The most frequently used codon of protein-coding genes was UAU. Isoleucine was identified as the most common amino acid, while proline was the least common amino acid in protein-coding genes. The gene-arrangement order is nearly the same when compared with other Ascomycota mitogenomes. Phylogenetic relationships based on the shared protein-coding genes revealed that E. album is closely related to the Cordycipitaceae family, with a high-confidence support value (100%). The availability of the mitogenome of E. album will shed light on the molecular systematic and genetic differentiation of this species.

ABSTRACT

Endocannabinoid receptor system is extensively expressed in the vertebrate retina. There are two types of cannabinoid receptors, CB1 and CB2. Activation of these two receptors by endocannabinoids N-arachidonoylethanolamide (anandamine, AEA) and 2-arachidonyl glycerol (2-AG) regulates multiple neuronal and glial ion channels, thus getting involved in retinal visual information processing. In this review, incorporating our results, we discuss the modulation of cannabinoid CB1 and CB2 receptors on retinal neuronal and glial ion channels and retinal synaptic transmission.

Objective To study the threshold effect of government health expenditure on residents' health.Methods Using threshold panel data model to perform the empirical research.Results There is threshold effect of government health expenditure on residents' health.There are two threshold values.As the increase of the proportion of the government health expenditure to the total fiscal expenditure,it has two changes of the impact on the health of residents.Conclusion The government should increase the health expenditure,and increase the proportion of the government health expenditure to the total fiscal expenditure,but the government should not blindly pursue the high input of health expenditure,focus on the efficiency of health expenditure,and avoid of resource waste.

ABSTRACT

Angelicae Sinensis Radix, with nourishing Yin, promoting blood circulation, and moisturizing dryness functions, is commonly used in clinical medicine. In order to investigate the effects and mechanism of Angelica sinensis(AS) on Th1/Th2 and Th17/Treg in mice with asthma and Yin deficiency syndrome, asthmatic and Yin deficiency syndrome Balb/c mice models were established by injecting and inhaling ovalbumin(OVA) and thyroxin. The models were treated with dexamethasone(DXM), AS extract and AS extract+DXM, respectively. Pathological examination of lung tissues was conducted by HE staining, and ELISA was used to detect the levels of IL-4, IL-17, IFN-γ, TGF-β as well as retinoic acid receptor-related orphan receptor (RORγt). Results showed that AS could significantly improve the situation of inflammation infiltration, increase ratios of IFN-γ/IL-4 and TGF-β/IL-17, decrease the levels of RORγt in lung tissues. The AS+DXM group showed a best treatment effect. The results indicated that AS played a therapeutic role for asthma with Yin deficiency syndrome and improved airway inflammation by inhibiting the expression of RORγt in lung tissues and regulating the balance of Th1/Th2 and Th17/Treg.

ABSTRACT

Glaucoma, the second leading cause of blindness, is a neurodegenerative disease characterized by optic nerve degeneration related to apoptotic death of retinal ganglion cells (RGCs). In the pathogenesis of RGC death following the onset of glaucoma, functional changes of glutamate receptors are commonly regarded as important risk factors. During the past several years, we have explored the mechanisms underlying RGC apoptosis and retinal Müller cell reactivation (gliosis) in a rat chronic ocular hypertension (COH) model. We demonstrated that elevated intraocular pressure in COH rats may induce changes of various signaling pathways, which are involved in RGC apoptosis by modulating glutamate NMDA and AMPA receptors. Moreover, we also demonstrated that over-activation of group I metabotropic glutamate receptors (mGluR I) by excessive extracellular glutamate in COH rats could contribute to Müller cell gliosis by suppressing Kir4.1 channels. In this review, incorporating our results, we discuss glutamate receptor- mediated RGC apoptosis and Müller cell gliosis in experimental glaucoma.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the clinical features of children with different clinical forms of congenital hepatic fibrosis (CHF), and provides a description of the characteristics of childhood CHF.</p><p><b>METHODS</b>Sixty children with CHF between January 2002 and June 2015 were enrolled, including 26 children with portal hypertensive CHF (PH CHF), 3 children with cholangitic CHF, 30 children with combined portal hypertensive and cholangitic CHF (mixed CHF), and 1 child with latent forms of CHF. The medical data of 26 children with PH CHF and 30 children with mixed CHF, including gender, age, clinical manifestations, physical signs, laboratory tests and imaging characteristics, were retrospectively studied.</p><p><b>RESULTS</b>Fever, jaundice and hepatomegaly were more frequently noted in children with mixed CHF than in those with PH CHF (P<0.05). Splenomegaly and liver cirrhosis occurred more often in children with CHF, but there was no significant difference in the incidences of splenomegaly and liver cirrhosis between the children with PH CHF and mixed CHF. The plasma prothrombin activity, white blood cell counts, platelet counts, mean platelet volume, serum levels of alanine transaminase, aspartate transaminase, alkaline phosphatase, γ-glutamyl transferase, leucine aminopeptidase, and total bile acids in children with mixed CHF were higher than in those with PH CHF (P<0.05). The decreased international normalized ratio and lower serum albumin levels were more frequently observed in children with mixed CHF than in those with PH CHF (P<0.05).</p><p><b>CONCLUSIONS</b>PH and mixed CHF are common forms in childhood CHF. The children with the two forms of PH usually manifest portal hypertension such as cirrhosis and hepatosplenomegaly. The liver damage may be common in children with mixed CHF.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical therapeutic effect and safety of application of Ilizarov technique combined with flap instant expansion technique in correcting tibia angular deformity combined with skin contracture by one stage.</p><p><b>METHODS</b>From January 2010 to January 2013, 30 cases of tibial deformity with skin contracture were corrected by Ilizarov technique combined with flap instant expansion technique at one stage, including 21 males and 9 females with an average age of(40.2±5.5) years ranging from 25 to 60 years. All patients underwent regular reexamination of X-ray. After removal of the Ilizarov external fixation, knee joint function were assessed by American Hospital for Special Surgery (HSS) scoring criteria, and the pain was evaluated by visual simulation score(VAS).</p><p><b>RESULTS</b>All patients were followed up for 6 to 35 months with an average of 22 months. Among them, the incision of 29 patients were primary healing, 1 patient had wound infection complicated by osteomyelitis, 2 patients complicated with fixed screw loosening, there were no expanded skin flap necrosis and neurovascular injury symptoms. The external fixators were removed at 4 to 7 months after operation with an average of(5.2±1.1) months. Correction angle was 10° to 35° degrees with an average of (25.5±3.5)°. HSS total score was 92.5±6.6 and the result was excellent in 25 cases, good in 4 cases, fair in 1 case; the VAS score was 1.2±1.5.</p><p><b>CONCLUSIONS</b>The application of Ilizarov technique combined with flap instant expansion technique is a good method for correction of tibial angular deformity with skin contracture by one stage, with a shorter time of external fixation frame, without skin necrosis and neurological symptoms, early load exercise and improve the limb function.</p>

ABSTRACT

The purpose of this study was to evaluate the effect and safety of Jinlong capsule combined with chemotherapy or radio-therapy for non-small cell lung cancer (NSCLS) using Meta-analysis. PubMed, Embase, CNKI and Wanfang databases were all searched without language restriction, and searching time was from January 1990 to July 2015. All eligible published studies were included in this study for quality assessment and data extraction. All the data were analyzed using Revman 5.3. A total of ten studies including 736 subjects (370 in Jinlong capsule plus chemoradiotherapy and 366 in chemoradiotherapy only) were finally included in this Meta-analysis. The result of Meta analysis showed that compared with pure chemoradiotherapy group, Jinlong capsule combined with chemoradiotherapy for NSCLC could improve the patients' curative effect (OR = 1.77, 95% CI: 1.29-2.43, P < 0.05), clinical benefit rate (OR = 1.89, 95% CI: 1.22-2.91, P < 0.05), life quality improvement rate (OR = 2. 56, 95% CI: 1.61-4.05, P < 0.05), and decrease leucopenia incidence rate (OR = 0.35, 95% CI: 0. 22-0.56, P < 0.05) and gastrointestinal reaction rate (OR = 0.67, 95% CI: 0.40-1.11, P < 0.05). The pooled results showed that Jinlong capsule combined with chemoradiotherapy for NSCLC could improve the curative effect and life quality, and decrease the adverse reaction of patients.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the relationship of bone cement distribution and the puncture angle in the treatment of thoracolumbar compression fractures with unilateral percutaneous kyphoplasty (PKP).</p><p><b>METHODS</b>The clinical data of 37 patients with thoracolumbar osteoporotic compression fractures underwent PKP between January 2013 to March 2014 were retrospectively analyzed, all punctures were performed unilaterally. There were 6 males, aged from 65 to 78 years old with an average of (71.83 ± 6.15) years; and 31 females, aged from 57 to 89 years old with an average of (71.06 ± 7.89) years. Imaging data were analyzed and puncture angle and puncture point were measured before operation. According to the measured data, the puncture were performeds during the operation. Distribution area of bone cement were calculated by X-rays data after operation. The effect of bone cement distribution on suitable puncture angle was analyzed; VAS score was used to evaluate the clinical effects.</p><p><b>RESULTS</b>The puncture angle of thoracic vertebrae in T8-T12 was from 28° to 33° with an average 30.4°; and the puncture angle of lumbar vertebrae in L1-L5 was from 28° to 35° with an average of 31.3°. Postoperative X-rays showed the area ratios of bilateral bone cement was 0.97 ± 0.15. Bilateral diffuse area were basic equal. Postoperative VAS score decreased significantly (1.89 ± 1.29 vs 7.03 ± 1.42).</p><p><b>CONCLUSION</b>Through measure imaging data before operation with PKP,the puncture point and entry point can be confirmed. According the measured data to puncture during operation, unilateral puncture can reach the distribution effect of the bilateral puncture in the treatment of thoracolumbar compression fractures.</p>
